Saliba’s Extended Care Pharmacy is hosting a Medication Assistant Training Class on Saturday, February 29, 2020 from 10 a.m. – 2 p.m.
The cost for the course is $40 per person and must be pre-paid. Your card will not be charged until the day prior to the scheduled event. Additional information about the course and a credit card authorization form are included below.
To register for the class, please complete both of the following steps:
1) Send an email to chilger@salibaspharmacy.com with the following information:
- Name
- Contact Number
- Valid email address
AND
2) Please fax your completed credit card authorization form to (623) 469-6262 by the registration deadline of February 22, 2020.
